Macquarie is obviously under the Macquarie Group umbrella, but the US operations are registered under Macqurie Holdings (USA), Inc. Which one should I use if applying to the US positions in the securities group?

Which worked fine, but Macquarie Group is technically correct as well. I think either of the first two are fine.
1 and 2 are fine, 3 isn't correct. You could also do Macquarie Capital Advisors. I recommend having anyone you can find from your network send it in though. Everyone in the NYC office knows the HR who does recruiting pretty well and you will get much better traction even just having it forwarded by an employee.
I'd go with 1 over 2 -- cover letter isn't a legal document. Putting "(USA), Inc" seems silly to me.
I think it's okay to just put "Macquarie Holdings" though.
Macquarie Holdings (USA) Inc. is just a holding company, nobody "works" for them.
Use Macquarie Group or Macquarie Capital Advisors.
