I do first name
first name if that's an English speaking country. I come from a country where we address people older than us by Mrs/Mr and I did that mistake by addressing my managers (very senior execs, MD equivalent) by Mr name surname. I can tell they found it amusing
MD: Bob Smith
Western english speaking countries: “Hi Bob”
More conservative but still english speaking places : “Dear Bob”
Asian/conservative countries where english is not the first language: “Mr. Smith”
